What is the D4 medical form?
The D4 medical form is a standard DVLA document used to record a driver’s medical information for Group 2 licence applications and renewals, including HGV, LGV, PCV, and C1. It is a paper form made up of structured sections and must be completed and signed by a GMC-registered doctor before being submitted to the DVLA.

How is the D4 form laid out?
Although the layout may vary slightly between versions, the D4 form is colour-coded and split into two parts.
Grey section — completed by the applicant (driver)
This section includes:
Front page
- Personal details
- Date of first licence (only applicable to those renewing their Group 2 entitlement)
- Driver’s registered GP details, including the name of the surgery, address, and telephone number
Note: even if you choose to have the medical completed by a third party rather than your own GP, you must still provide your registered GP’s details. This information is required if the DVLA needs to contact your GP to obtain your full medical records.
Bottom of each page
- Your full name and date of birth must be written in the designated grey box at the bottom of each page.
Last page (Page 8)
- Driver declaration and consent for the DVLA to contact your GP to obtain full medical records.
Please be aware that refusing to give consent will usually result in refusal of a provisional licence, as the DVLA must review medical records before making a final decision.
Green section — completed by a GMC-registered doctor
This section includes:
- Structured medical information covering the brain, heart, mental health, substance misuse, and sleep apnoea
- A section for eyesight information
- Clinical observations and comments
- Doctor’s stamp, signature, and GMC number
Note: unsigned or undated sections are among the most common reasons D4 forms are rejected or returned.
How long is the form valid for?
The D4 medical form is valid for up to four months from the date of completion by the doctor.
