We provide D4 medical assessments for drivers applying for or renewing their DVLA Category D or D1 licenses.

What is a Bus Medical?

The DVLA mandates that any driver planning to drive a passenger carrying vehicle (PCV) or a public service vehicle (PSV) carrying 8 passengers or more must undergo a medical assessment to ensure their fitness to drive.

What is involved in a Bus medical?

The DVLA has a standardised medical form called the D4 Form, which serves the basis of the assessment:

  • Weight & Height measurements
  • Smoking and alcohol history
  • Vision assessment
  • Blood Pressure & Pulse check
  • General Musculoskeletal (MSK) assessment
  • Medical conditions evaluation

What is the Bus Medical Requirements?

The DVLA classifies drivers into Group 1 and Group 2 categories. Lorry and bus drivers fall under Group 2 drivers, who must meet stringent medical criteria to minimize the risk of harm.

Vision

Read car plates from 20 meters. No double or single-eye vision*

Blood Pressure

Should be below 180/100 mmhg, otherwise cannot operate the vehicle

Physical Strength

A good level of musculoskeletal (MSK) function is required to drive safely

Health Conditions

Should be well-controlled according to DVLA Driving Fitness Guidance.

Grandfather Rule: Exemption for single-eye Bus drivers with licenses obtained before 1 January 1991, provided the condition was declared at the time.

How often do you need a Bus Medical test?

Medical assessments are required at the initial application, followed by five-year evaluations from the age of 45 until 64.

For drivers aged 65 and above, an annual D4 medical assessment is necessary for those wishing to maintain their license entitlements.

You can obtain a D4 form free of charge either by requesting it directly from the DVLA or by visiting a post office. If you encounter any issues, just let us know, and we can arrange it for you at no extra cost.
Note: The D4 is valid for 4 months from the date it is signed and dated by the doctor and optician or optometrist.
