We provide medical tests for paramedic and ambulance drivers wishing to obtain or renew their DVLA C1 category licenses.

What is an Ambulance Medical?

The Secretary of State for Transport’s Medical Advisory Panels advised the DVLA that all drivers, including paramedics and ambulance drivers, operating vehicles weighing more than 3.5 tonnes, should undergo a specific medical assessment known as the D4 medical.

New ambulance and paramedic drivers must submit the D4 assessment to obtain a provisional C1 license before starting practical lessons.

What is involved in an Ambulance Medical?

The C1 Medical is completed on a special form provided by the DVLA known as the D4 Form. The assessment consists of:

Weight & Height measurements
Smoking and alcohol history
Vision assessment
Blood Pressure & Pulse check
General Musculoskeletal (MSK) assessment
Medical conditions evaluation

What is the Ambulance Medical Requirements?

The DVLA classifies drivers into Group 1 and Group 2 categories. Ambulance drivers who operate vehicles weighing over 3.5 tonnes fall under Group 2, and must meet stringent medical criteria to minimize the risk of harm.

Vision

Read car plates from 20 meters. No double or single-eye vision*

Blood Pressure

Should be below 180/100 mmhg, otherwise cannot operate the vehicle

Physical Strength

A good level of musculoskeletal (MSK) function is required to drive safely

Health Conditions

Should be well-controlled according to DVLA Driving Fitness Guidance.

Grandfather Rule: Exemption for single-eye ambulance drivers with licenses obtained before 1 January 1991, provided the condition was declared at the time.

How often do you need an Ambulance Medical test?

At initial application: In order to obtain your C1 provisional license.
At age 45: a D4 will be needed every 5 years until age 65.
At age 65 and onward: Annual assessments.

Please note that drivers with medical conditions such as diabetes or Sleep Apneoa may be required to undergo their medical assessments more frequently, typically every 1 to 3 years.

You can obtain a D4 form free of charge either by requesting it directly from the DVLA or by visiting a post office. If you encounter any issues, just let us know, and we can arrange it for you at no extra cost.
Note: The D4 is valid for 4 months from the date it is signed and dated by the doctor and optician or optometrist.
