1. Introduction
These Terms and Conditions govern bookings and medical services provided by Dr4D4 (“we”, “us”, “our”). By booking an appointment with Dr4D4, you agree to be bound by these terms.
Our driver medical assessments are conducted in line with current DVLA/Group 2 medical standards and are intended to assess fitness to drive.
2. Booking and Appointment Confirmation
2.1 Appointments can be booked online or via other approved booking methods.
2.2 A booking is only confirmed once you receive confirmation by email or SMS.
2.3 We reserve the right to decline or reschedule appointments where necessary.
3. Your Responsibilities
You are responsible for:
- Booking the correct type of medical assessment
- Bringing valid photo identification (driving licence or passport)
- Bringing your DVLA/local authority medical form where required
- Providing accurate medical history and medication details
- Bringing glasses/contact lenses and optical prescription if worn for driving
- Providing GP details where requested
Failure to bring required documentation may result in cancellation of the appointment and charges may still apply.
4. Medical Assessment
4.1 Medical examinations are screening assessments only and are not a substitute for full GP or specialist review.
4.2 We complete forms based on the information and measurements obtained at the time of examination.
4.3 We cannot guarantee licence approval by the DVLA or any licensing authority.
4.4 Any further investigations required by DVLA or licensing bodies are the responsibility of the client.
5. Eye Tests and Blood Pressure
5.1 Vision screening is performed to DVLA standards but is not equivalent to a full optician examination.
5.2 Blood pressure readings are recorded as measured at the appointment.
5.3 Elevated readings may lead to appointment resechuling until blood pressure is within normal limits.
6. Language and Communication
If there is a language barrier, you must arrange a suitable translator to attend with you.
We reserve the right not to proceed if adequate communication cannot be achieved. No refund will be issued in such cases.
7. Deposit & Cancellation Policy
A £10 non-refundable booking deposit is required to secure all appointments.
- Cancellations with more than 24 hours’ notice – deposit may be transferred to a new booking.
- Cancellations with less than 24 hours’ notice – deposit will be retained.
- Failure to attend – deposit will be retained.
- Rescheduling within 24 hours – treated as a late cancellation.
The remaining balance is payable on the day of the appointment.
8. Late Attendance
If you arrive late and we are unable to complete your appointment, this will be treated as a non-attendance and charges will apply.
9. Cancellation by Dr4D4
If we need to cancel or rearrange your appointment (for example due to clinician illness or unforeseen circumstances), we will:
- Offer the next available appointment, or
- Refund any monies paid (limited to the appointment fee)
We are not liable for any consequential losses.
10. DVLA or Licensing Authority Queries
10.1 It is your responsibility to check all forms before submission.
10.2 If the DVLA or licensing authority requests amendments, you must notify us promptly.
10.3 Additional appointments may be chargeable where re-examination is required.
11. Limitation of Liability
To the fullest extent permitted by law:
- We do not guarantee licence approval.
- Our liability is limited to the fee paid for the medical examination.
- We are not responsible for indirect or consequential losses.
Nothing in these terms excludes liability for death or personal injury caused by negligence.
12. Data Protection
Your personal and medical data will be processed in accordance with UK data protection legislation and our Privacy Policy.
